2. We enroll your patients and deliver their devices. KAREVitals’ complimentary concierge will connect to your EHR, and automatically identify patients who are eligible for the remote patient monitoring program, and perform patient consent and training. We ship devices directly to your patients fully configured and ready to use.
3. Respond to critical alerts. The system ensures that patients are testing daily. Notifications about abnormal readings will be sent to you for urgent attention.
4. Review patient data. KAREVitals’ generates data from your patient’s device to your Practitioner’s Portal Account. Every month, we email you to review your patient's data and to make sure you meet the Medicare requirements for billing.
5. Get paid. You bill the insurer (i.e. Medicare) based on your standard billing procedures for CPT Codes 99453, 99454, 99457, and 99458.

The Remote Patient Monitoring program's success depends on seamless integration with Electronic Health Records (EHR). This integration will allow for seamless integration into existing workflows and for quality reports to be generated quickly and efficiently through the data collected through RPM.KAREVitals™ will provide EHR integration upon request, but some Electronic Medical Records systems that we already have integration with are as follow:

When you sign up with us, we will ask you to set your thresholds for each physiological parameter at "At Risk" and "Critical". These are the practice-wide default thresholds.
You will also have the option to create personalized thresholds for each patient. When a patient has an At Risk reading, we review and assess the reading to figure out whether it is affecting their health or if a phone call is needed.
If a medical patient has a Critical reading, we will call the patient and the ordering provider to establish what needs to be done. Phone calls are secure and recorded so there is a paper trail of everything that happens.

When these reimbursements are added together, the amount a single remote patient monitoring patient can earn a practice is up to around $207 per month.
More often than not, a practice will typically furnish about 20 minutes of RPM care management per month to a patient, which will bring the total monthly reimbursement to around $120 per Medicare beneficiary. That's nothing to scoff at.
